What kind of report can I make in NYC if I get injured involving anything other than criminal matter or motor vehicle accidents?

If you are involved in an incident where you get injured by non-criminal matter or non-motor vehicle accidents, call 911 and request for an ambulance or EMS. Explain in detail to 911 operator about what happened and what caused the injury.

Once the ambulance arrived, the EMTs will evaluate the injury and then they might treat you on scene or take you to the hospital depending on injury. If police also shows up, you can request the officer to make an “Aided Report” for the record.

If police don’t show up or EMTs take you to the hospital before police arrived, keep the hospital discharge papers once you get discharged. Then, go to the nearest NYPD precinct with hospital discharge papers. Look up here to see which precinct is near your residence or the place where the incident happened. When you are welcomed by a uniformed officer or a civilian member in the precinct, explain them what happened and give them hospital discharge papers and tell them that you wish to make an “Aided Report”.

Remember, either on scene or at the precinct, officers will take your information to do the report. Also, take pictures where the incident happened (for example, hole on the sidewalk, construction site etc.) and pictures of your injuries. Once the officer has taken your information, s/he will give you an incident slip or their name, badge number, and the precinct number. Later, you can request a copy of the “Aided Report” using those information.

You can request or obtain a copy by visiting here. Once there, scroll down to see the process to obtain a copy of the “Aided Report.” Few examples below:

  1. You get injured at your workplace.
  2. You fell on the sidewalk because of any defects on the sidewalk; for example, cracked or uneven sidewalks, hole in the sidewalk etc.
  3. You fell because of snow on the sidewalk or street.
  4. You get injured inside one of the NYC subway stations, staircases, mezzanine, inside train etc.

Basically, if you get injured in a non-criminal matter or non-motor vehicle accident, you can make an “Aided Report” to NYPD.
